SERVICES PROVIDED BY TREASURED MOMENTS

PROFESSIONAL MONITORED VISITATION OFF-SITE:

There are numerous activities which can be scheduled to facilitate interaction between parent and child(ren) with little to no cost.

Suggested locations below:

* Museums

* Play grounds

* Restaurants

* Local Parks

* Library

SUPERVISED EXCHANGE AVAILABLE (3 kinds of exchange)

* Face-to-Face: There is no contact between the two parties. Transfers of the child only with provider within    ear shot and eye shot.

* Curbside: Child(ren) are dropped off by one party at specific location without exiting vehicle.

* No Contact: Between both parties, there is no contact of any kind during the exchange.

         All exchanges are observed and documented by provider

INTAKE PROCESS

Before TMVS can render visitation services, the intake appointment must be established. The intake process is when TMVS has the pleasure of meeting the custodial party and non-custodial party for the first time in person, on SEPARATE APPOINTMENTS, to discuss the nature of your visits.

Below are a list of items that will be addressed during the process:

* Complete Intake application

* Review court orders

* Schedule visitation appointments for (NCP)

* Review method of payment and fees

* Review Service Agreement

* Safety and Security Measures

* Confidentiality

Allow approximately 1 hour to complete this process.

REPORTS AND DECLARATIONS

We provide the following reports: Detailed, Summary and Termination.

All reports can be obtained upon request from parent, court, and attorneys.

COURT APPEARANCE

Court appearance can be received upon subpoenas from court or attorneys.

Must have 15 days notice prior to court date.

TMVS does not convey or communicate to Parties' Attorney, Counsel or Law at any time.
